The McKenzie Method, also known as Mechanical Diagnosis and Therapy, is a philosophy of active patient treatment and prevention of spinal pain. The method was developed in the late 1950’s by Robin McKenzie, a physical therapist from New Zealand. McKenzie noticed that some of his patients experienced symptomatic relief from back, neck, and extremity pain when the spine was extended, or bent backwards. For these patients, radiating pain seemed to shift from an arm or leg and settle in the middle of the spine, relieving the majority of their discomfort.
Based on these occurrences, he coined the term “centralization phenomenon,” and developed his world-renowned philosophy of encouraging those in pain to practice extension positions and other movements to encourage centralization. The method focuses on reducing pain, avoiding risks that cause discomfort, becoming comfortable enough to return to regular activities, and incorporating certain lifestyle changes to promote self-maintenance.
Is the McKenzie Method Right for Me?
Not all painful back or neck conditions can be treated by the McKenzie Method, so it is important to consult with a certified McKenzie practitioner before starting specific exercises or movements. Before treatment and preventative measures are suggested, a practitioner will begin a process to determine whether a patient can truly benefit from the method, which includes assessment and classification into the following three mechanical syndromes:
- Postural – soft tissue stress causes pain in certain positions, but no permanent damage has occurred
- Dysfunction – stressed scar tissue from a previous injury causes pain, but no active injury is present
- Derangement –shifting inner intervertebral disc material results in vertebral movement and the development of pain
